Online forms

There are two types of online forms on the website and they are processed differently.

• All of the forms are available via the clinical software and you need to log in (you will need to register first).
  • When logged on our questionnaires are located under the Records tab
  • To complete a questionnaire when logged into systmonline you will need to do this via computer or laptop (not available on mobile devices)
    • You need to use:
    SystmOnline Access
    
    

• Some can be done directly without login by filling in your details and submitting via this website.These forms can be noted on the right-hand side menu and below clearly marked (Blue text, to click in).

It includes:

• Vasectomy referral.

General health (Like Diabetes risk, Lifestyle, health check)

It includes:

• Diabetes risk assessment
• Lifestyle assessment
• New patient health check

General queries (Like appointment cancel,Referrals, Sick note, Medication query)

It includes:

• Cancel an appointment
• Fit note
• Medication query
• Referrals to hospital issues
• Sick note
• Speed up appointment need.

Infections (Like Athlete’s foot, BV, thrush)

It includes:

• Athlete’s foot
• Bacterial vaginosis
